TERMS OF SERVICE (CONTACT FORM)
§ 1. Definitions
Usługodawca —

Usługi — bezpłatne korzystanie z interaktywnego Formularza w celu kontaktu z kliniką w sprawach dotyczących jej działalności.
Pacjent — osoba fizyczna korzystająca z ww. Usług.
Umowa — stosunek prawny pomiędzy Pacjentem a Usługodawcą, powstały w wyniku wypełnienia i wysłania Formularza.
Formularz — interaktywny formularz dostępny na stronie internetowej Usługodawcy, umożliwiający Pacjentowi zadanie pytania lub przesłanie zapytania do kliniki.
§ 2. Terms of Service
To use the Services, you must have:

access to the Internet;

a current web browser (Chrome, Firefox, Safari, Edge);

an active email address.
The Patient is solely responsible for ensuring the technical compatibility of their device and the protection of personal data (e.g. antivirus software, firewall, etc.).
The Services provided through the Form are free of charge.
The Form may be used by both registered and non-registered users of the clinic.
Creating an account is not required.
§ 3. Forms
To submit an inquiry via the Form, the Patient must provide the following information:

First and last name

Email address

Content of the inquiry

Confirmation of having read these Terms of Service

Confirmation of having read the Privacy Policy
The Patient is obliged to provide only truthful and up-to-date personal data.
It is prohibited to use pseudonyms, false or third-party data, as well as to transmit harmful or unlawful content.
The Agreement is considered concluded at the moment the Form is submitted.
It terminates upon the provision of a response to the inquiry.
Response time:
The Service Provider undertakes to respond to the inquiry within 7 business days.
The response is sent to the email address provided.
If a phone number is indicated, the response may also be provided by phone.
§ 4. Complaints
The Patient has the right to submit a complaint regarding the quality of the Services.
Complaints are accepted in writing:

or by email: lodz@identa.com.pl
The complaint should include:

Patient’s first and last name

Contact details (email or postal address)

Description of the situation

Clearly stated demands
The complaint will be reviewed within 14 calendar days from the date of its receipt.
If the complaint is incomplete, the review period begins once the full information is received.
§ 5. Liability
The Service Provider is not liable for:

delays or lack of response due to reasons beyond the clinic’s control;

violations of these Terms by the Patient;
-
the provision of false, incomplete, or third-party information;

malfunctions caused by technical failures on the Patient’s side or by force majeure (accidents, natural disasters, etc.);
-
damage caused by malicious software on the Patient’s device.
§ 6. Personal Data

The Service Provider is the controller of the personal data processed in connection with inquiries submitted through the Form.

The processing of data is carried out in accordance with the provisions of the General Data Protection Regulation (GDPR).

§ 7. Final Provisions

The provisions of the law of the Republic of Poland shall apply to this Agreement.

The Service Provider reserves the right to amend these Terms.

The updated Terms enter into force upon their publication on the website and apply to Agreements concluded after that date.
