Regulations on the Purchase, Use and Refund of Vouchers and Deposits

1. General Provisions
These regulations define the rules for the purchase, use and refund of vouchers and deposits offered by Identa Centrum Dentystyczne Sp. z o.o.
The purchase of a voucher or payment of a deposit constitutes full acceptance of these regulations.
The regulations are available on the clinic’s website and at the reception desk.
2. Definitions
Voucher — a document confirming prepayment for future dental services at the Identa clinic.
Deposit (advance payment) — an amount paid toward a future service.
The voucher and deposit do not constitute a security deposit within the meaning of the Civil Code unless otherwise stated in writing.
A voucher is not a fiscal receipt or an invoice.
3. Purchase and Payment Methods
A voucher or deposit may be purchased:

on-site at the clinic (cash / card / BLIK),
-
online (if such an option is available).
In the case of cash payment, the client receives a fiscal receipt, the safekeeping of which is solely the client’s responsibility.
The clinic does not issue duplicate fiscal receipts.
At the client’s request, the clinic may issue a payment confirmation, which:

is not a fiscal document,
-
does not replace a receipt,
-
does not entitle the client to a cash refund.
4. Right of Withdrawal (14 Days)
The right to withdraw from the contract within 14 days applies only in the case of vouchers purchased online or remotely, in accordance with consumer protection law.
This right does not apply to vouchers purchased on-site at the clinic.
In the event of a valid withdrawal, the client is obliged to return the unused voucher.
5. Refunds
5.1 General Rules
Refunds are possible only in cases provided for in these regulations.
The clinic reserves the right to refuse a refund in situations not covered by the regulations.
5.2 Cash Refunds
Cash refunds are possible only upon presentation of the original fiscal receipt.
The absence of a fiscal receipt unequivocally excludes the possibility of a cash refund, regardless of circumstances.
Refunds are made using the same payment method as used for the purchase, where possible.
6. No Right to Refund
The following are not subject to refund:

vouchers purchased on-site at the clinic,
-
cash payments without presentation of the original fiscal receipt,
-
partially used vouchers,
-
expired vouchers,
-
lost or damaged vouchers.
7. Voucher Validity Period
Each voucher has a validity period specified at the time of purchase.
The voucher validity period is not automatically extended.
An extension is possible only upon individual agreement with the clinic and is not an obligation of the clinic.
8. Change of Voucher Purpose
A voucher may be:
-
used for a different service,
-
transferred to another person,
subject to prior agreement with the clinic.
The clinic reserves the right to refuse a change of voucher purpose in justified cases.
9. Client Responsibility
The client bears full responsibility for the safekeeping of:
-
the voucher,
-
the fiscal receipt.
The clinic is not responsible for the consequences of lost documents.
10. Legal Basis
Cash refunds without presentation of the original fiscal receipt are not permitted under applicable tax law. A clinic operating a fiscal cash register is not entitled to issue refunds based on copies of documents, client statements, or internal payment confirmations.
11. Final Provisions
The clinic reserves the right to amend these regulations.
In matters not regulated herein, Polish law shall apply.
These regulations enter into force on the date of publication on the clinic’s website.
